Introduction.
Our Extended Scope Practitioner service provides patients with a comprehensive assessment of orthopaedic / musculoskeletal conditions and effective pathway management, both in a primary and secondary care setting.
Using evidence base knowledge, a detailed physical assessment is carried out in clinic. Investigations and imaging can be requested to correctly identify a problem(s). These can include bloodwork for suspicion of infection, for example. The professionals have up to date training which allow them to reliably request X-rays, Ultrasound and MRI’s, depending on clinical reasoning and symptom presentation. An evidence-based management strategy can then be determined. This can include conservative measures, such as physiotherapy referral, injection therapy, lifestyle advice and dietician referral. A referral for a surgical opinion can be made when either initial conservative measures fail or the professional deems a surgical option to be the most appropriate. When surgery for the problem is unavailable on island (for example spinal issues, hip arthroscopy, etc), off island referrals are made.
Referrals to the pain management service are offered when standard conservative management is ineffective and the patient presents either chronic pain or more extreme pain. Regular interaction and discussion of complex clinical cases with the Orthopaedic Surgeons and Pain Management consultants enhance the professional’s clinical decision making.
