The role of the Rapid Response and Reablement Team is to reduce delayed discharges, to improve the early supported discharge of patients to their own home, to prevent unnecessary admissions to hospital and to provide increased levels of support for early discharges into the community. The post holder will provide highly specialised physiotherapy, in conjunction with the Rapid Response and Reablement Team patients within their own homes and across various community settings. The post holder will be involved in conducting physiotherapy assessments, treatment and physiotherapy diagnosis including identifying and discussing appropriate medical interventions.
Responsibilities of the role:
- Provide the provision of rehabilitation to a diverse range of clinical conditions and patient types in a variety of environments.
- Assist in the prevention of unnecessary hospital admissions via the Emergency Department alongside Rapid Response.
- To co-lead in the implementation of service change and development of the Rapid/ Reablement Team.
- Assist in the facilitation of hospital discharge and reduce packages of care.
- Provide community reablement for patients who are discharged early.
- Partake in service planning and service and staff development within a multidisciplinary framework.
- To work as part of the Rapid/Reablement Team undertaking the management of patients requiring reablement using developed highly specialist knowledge.
- Work closely with MDT colleagues to review, co-ordinate and ensure consistency of the Rapid Response and Reablement service, including prioritisation and allocation of resources to meet service demands and pressures.
- Assist in the evaluation, audit and development of clinical practice, activity and effectiveness of the Rapid Response and Reablement service delivery to support the clinical governance strategy and enhance patient care.
- Provide physiotherapy input to Multi-Disciplinary Team and nursing and residential care home staff, including discussions and decision-making to maximise patient care and, where appropriate, discharge planning.
- Act as a resource to provide education for nursing/support workers within social care staff and other healthcare professionals and physiotherapy colleagues at GTG
- Make decisions and judgements on the safety and suitability of the patient’s home environment regarding the modification of treatment and use of physiotherapy equipment.
- Liaise with GTG Lead Rehab Physiotherapist/Head of Clinical services regarding service needs and developments.
- Manage own caseload; being able to respond to change in caseload and patient needs. Demonstrate the ability to prioritise as necessary.
- Formulate and deliver an individual physiotherapy treatment programme based on highly specialised knowledge of evidence-based practice and treatment options using clinical assessment, outcome measures, excellent reasoning skills and knowledge of treatment skills e.g. therapeutic exercise, manual physiotherapy techniques, patient education.
- Promote high standards of clinical care, working as an autonomous highly specialised physiotherapy practitioner.
- Supervise and delegate treatment programmes to support workers.
- To deliver education, advice and training of patient carers and external agencies as part of care packages as required.
- Provide highly specialised clinical skills in rehabilitation physiotherapy including relevant equipment.
- Provide, as an integral member of the inter-disciplinary team, ongoing support for the patient and their family / carers during the process of rehabilitation.
- Participate in education and training of students.
- Make referrals to other support/services as appropriate.
- Report on stock levels and equipment needs as required.
- Maintain cleanliness and tidiness of working environment.
- Create, maintain and update patient records and reports according to professional standards and GTG and HSC policy.
- Update and maintain Trak system.
- Participate in the emergency respiratory on-call rota.
- Participate in the implementation of policy and service development changes to GTG’s and team’s working practices and initiate and implement changes to working practices where this will result in improved performance.
- Cross-cover other clinical areas when service needs demand.
- Participate in supervision, training and education of junior staff.
- Participate in induction (including buddy system) and training of new staff when required.
- Participate in planning and delivering training for physiotherapy staff as part of the in-service training programme.
Qualifications:
- Degree or equivalent in Physiotherapy.
- Evidence of attendance on specialist post-graduate courses relevant to the clinical area.
- Registered Physiotherapist (HCPC).
- Member of the Chartered Society of Physiotherapy (CSP).
- Documented evidence of continuing professional development.
- Evidence of active participation in in-service training programmes.
- Valid driving licence.
Experience:
- Highly specialised knowledge and experience of a range of approaches to the management of patients requiring rehabilitation (essential).
- Previous experience of work in inter- and multidisciplinary teams to include community settings (essential).
- Experience supervising other staff or students (essential).
- Minimum 5 years post-graduation (desirable).
- Minimum 2 years post-graduate experience within specialised service area (desirable).
- Knowledge or practical use of audit (desirable).
- Experience of project work and identifying and implementing evidence-based practice (desirable).
